Moving towards best practices in Community Stroke Rehabilitation

As a part of a multi-year initiative to establish a new, comprehensive community stroke rehabilitation program in Ontario, Ontario Health - CorHealth partnered with Heart & Stroke to conduct a current state assessment of Community Stroke Rehabilitation (CSR) services. This assessment was comprised of program surveys and engagement sessions with providers and people with lived experience. The aim was to identify program limitations and critical resources needed. Planning has begun to work with local programs in the South East to identify quality improvement initiatives that will help to address priority gaps identified in our region.  

Several key project milestones were met during year 1 of the multi-year CSR project led by CorHeatlh Ontario. The southeast region has been actively involved in the CSR model of care project. The region has contributed to the development of the model of care  and participated in the current state gap analysis completed during the winter of 2023. Providence Care Hospital implemented a successful pilot outpatient CSR program with temporary funding allocated in fiscal year 2022-23. This program is featured in the photo above. The region awaits news on further funding.

Community Stroke Rehabilitation providers, hospital stakeholders, and stroke survivors attended a South East presentation on August 15 that reviewed results of the current state gap analysis conducted in the winter of 2023. Planning is underway for local teams in the southeast to meet in the fall of 2023 to review opportunities identified in the current state gap analysis to align practices with the CSR model of care.
