This in-person workshop was held at the Brockville Country Club, bringing together health care providers from primary care and Brockville General Hospital, including the Stroke Prevention Clinic. This session offered all attendees an opportunity to learn about each other's roles, and to build new (or revive pre-existing) connections across the Leeds and Grenville part of Southeastern Ontario.
