Join us for the live webinar about the Community Paramedic Post-Stroke and TIA Program, an in-home service where paramedics work with patients/families to improve safety, recovery, and quality of life after stroke. Up to 80% of strokes are preventable and this program helps to support personalized plans to reduce future stroke risk.
Objectives of the session:
- Review any questions that that emerged from the 9 online modules
- Explain the implementation process for a TIA/post-stroke visit with patients/family/caregivers
- Determine appropriate resources to guide TIA and stroke care based on best practices in Leeds and Grenville counties
