Stroke Network of Southeastern Ontario Congratulates Brockville General Hospital on Community Stroke Rehab Program Launch
Congratulations to Brockville General on the successful launch of its new Community Stroke Rehabilitation Program. The first patient was seen on March 30, 2026! This important milestone expands access to high‑quality, interdisciplinary stroke rehabilitation services for individuals living in the region.
The program is delivered by a dedicated multidisciplinary team, including Sarah Nichol, Project Coordinator and Physiotherapist (PT); Robyn Valentine, Team Navigator and Occupational Therapist (OT); Brihana Marcel, Speech‑Language Pathologist (SLP); Leanne Purdie, Social Worker (MSW/RSW); Kasey Andrews, Occupational/Physiotherapy Assistant (OTA/PTA) and program manager Jessica Matysiewicz.
Brockville General’s Community Stroke Rehabilitation Program provides intensive physiotherapy, occupational therapy, speech‑language therapy, and social work services to individuals recovering from stroke. Currently, patients are referred to the program following discharge from Brockville General’s hospital settings, including inpatient acute care and inpatient rehabilitation.
The primary goal of the program is to support people living with stroke to return to what matters most to them. Guided by the Provincial Community Stroke Rehab Model of Care. Services are individualized based on each patient’s unique goals, priorities, and rehabilitation needs, and may involve one or several members of the interdisciplinary team. A key element of this Model of Care is the intentional integration of healthcare system navigation support for individuals and their family or support persons.
To further strengthen quality and accountability, the program is incorporating patient‑reported experience measures (PREMs) and patient‑reported outcome measures (PROMs). These tools help evaluate service effectiveness while demonstrating the meaningful impact community‑based stroke rehabilitation can have on recovery and quality of life. The Stroke Network looks forward to supporting the Brockville General team as they review and learn from these outcome measures.
With the addition of Brockville General’s Community Stroke Rehabilitation Program, Southeastern Ontario now has four hospital‑based Community Stroke Rehabilitation programs: Quinte Health, Providence Care Hospital, Brockville General, and the Perth and Smiths Falls District Hospital. For individuals who require rehabilitation services in their homes, Ontario Health atHome continues to provide Community Stroke Rehabilitation services across the region following hospital discharge.
