New Provincial Resources Released to Enhance Implementation of Secondary Stroke Prevention Best Practices

A provincial workgroup of clinical experts was tasked with updating the Stroke Prevention Clinic Model of care in Ontario. The workgroup identified resources including strategies to implement best practice core elements in Secondary Stroke Prevention Clinics and to enhance system-wide performance and outcomes for people with TIA and stroke. The work has been completed and the compilation of resources was released on January 30th. The new resources and final report on this work can be accessed here.

Resources include:

  • Stroke Prevention Clinic (or equivalent) Core Elements ¬†along with a self-assessment tool
  • A Toolkit of practical resources and strategies to help implement the provincial core elements. The toolkit was informed by self-assessment results from Stroke Prevention Clinics across Ontario. Many resources from our region were included
  • Quality indicators relating to stroke prevention
