Holding Back the Floodgates: Approaches to Stroke Prevention

This workshop is intended for the Interprofessional team members of Primary Care and Stroke Prevention Clinics, including but not limited to: Physicians, Nurse Practitioners, Nurses, Allied Health professionals, Paramedics, Health promoters and Medical Secretaries in Southeastern Ontario.

 Clinicians and other team members will learn from each other and look for ways to further support the prevention and care of TIA and stroke patients in your local communities. Through the review and discussion of best practices, latest evidence and case studies you will be able to:

•Describe new best practices for vascular health promotion, secondary stroke prevention and post-stroke care strategies to use in the Primary Care setting and Stroke Prevention Clinics.
•Describe the management of the TIA and stroke patient including assessment, referral and triage processes, treatment and management.
•Identify practical ways to collaborate with patients, families and other health professionals to improve vascular health and management of risk factors.
•Discuss common needs and identify opportunities to enhance care in your local community.
•Further develop a collaborative network of health care providers within your local community.
•Engage in interactive learning that supports a practical, interactive and problem-solving approach.
