On October 24, 2025, the Stroke Network hosted the Community Stroke Rehabilitation (CSR) Forum, bringing together community therapists, rehabilitation assistants, and managers from across the region. The goal of this event was to share learnings from CSR related work within the region and to highlight clinical insights that support the CSR Model of Care. A visiting CSR team from Sudbury joined the day and generously shared their experiences and approaches to deliver the CSR Model of Care with a patient centred approach embracing individualized strategies for goal attainment and community re-integration.

  • The forum also featured two guest speakers. Tamalea Stone, an Occupational Therapist with specialist certification in Driving Assessment, presented on return to driving after stroke. Shannon Mulholland, a Physiotherapist with extensive community stroke experience, delivered an engaging session on high-level balance assessment and treatment, focusing on strategies to help clients achieve advanced functional goals and enhance participation in daily and community life. The day was completed with a series of Regional CSR “Brag and Steal” Presentations

A total of 54 participants attended, representing all community stroke rehabilitation programs across the region, including Brockville General Hospital, who will soon be launching their own CSR program. Participants shared that the opportunity to gather as a regional community of providers was extremely valuable. All evaluation respondents indicated a strong interest in repeating the forum in person—either annually or every other year. Feedback described the event as “excellent,” “very informative,” and “a great opportunity to connect with other agencies and providers.”

NEW Returning to Driving Resource: When a patient’s medical condition is reported to the Ministry of Transportation (MTO), the path to returning to driving can feel overwhelming and uncertain. Therapists play a crucial role in helping patients navigate the required steps to regain independence while maintaining safety for both the individual and the community. With permission from the Southwestern Ontario Stroke Network (SWOSN), the Stroke Network of Southeastern Ontario (SNSEO) and the Providence Care Hospital OT (special thanks to Zanna Farmer) adapted SWOSN’s 2024 Driving After Stroke Resource. This new resource aims to support therapists across the stroke continuum in guiding patients through this process with clarity and confidence.
